About the network
Heartland Community Health Network supports health centers in Iowa, Nebraska, and Texas by fostering collaboration and helping them deliver primary medical, dental, and behavioral health services. Since its start, Heartland has grown into a network of ten centers, showing its ability to meet diverse community health needs.
The organization’s growth has been strategic, focusing on shared services like bulk purchasing, credentialing, and clinical guidelines to improve efficiency and lower costs. Investing early in the NextGen Electronic Health Record (EHR) system laid a strong foundation for long-term success.
Since Heartland’s beginning in 2002, we have focused on developing customized tools, services, reporting and data analytics for our members. Heartland has focused on aligning with like-minded Community Health Centers, prioritizing collaboration, continuous improvement, and impactful patient care.
Heartland’s award winning development team has created customized tools and reporting so members can provide the very best in quality to their patients. Heartland hosts NextGen EHR in their data center in Nebraska and provides data analytics to help identify care trends, support evidence-based decisions, and reduce health disparities. The network’s IT services—including system implementation, data security, disaster preparedness, helpdesk support, and training—give member centers the infrastructure needed for efficient, high-quality care.
Financially, Heartland relies on three main revenue sources: member fees, IT services for non-members, and grant funding—especially from HRSA. In 2023, the network’s health centers served over 142,000 patients in urban, rural, and frontier areas, supported by advanced health IT systems that improve care delivery and outcomes.
